depends on the company, position applying for, etc. is it a security company or just a typical office company.

most state they will do one but never do it. at most they normally will run a police check to see if any felonies or warrants. some might run a BMV check to see if you are a high risk for auto insurance if you will be driving any of their vehicles. some go as far as calling past employers and even your references and maybe your neighbors.

most don't do much past saying they will do one or a basic police check. costs too much money and man hours to go sniff around on every person they are thinking of hiring. normally its put in to scare people away that have issues.

just like some biz's state a drug test will be done yet one is never done. this scares away all the crack addicts from the job without the expense of testing each person.

basic office.....wouldn't sweat it unless you have felonies, etc. security job....bend over and expect to get probed.

I do not know about all companies, but in our company you have to sign agreement for the check and it tells you what they will be checking for. In earlier jobs I have had, it has been generally in the same way. It varies from company to company, depending on the requirements for that company, but usually they will list out what they are going to check for and you have to sign and agree to it before they can do it.
