the usual reaction would be:
the one who walks in: oh, sorry!
the one inside: eeeeeeek..!
the one who walked in should have knocked, no matter. i always do. and the one inside should have locked the door. personally, if the doorlock is broken and i need to pee badly and have no other choice but to do it there, i'll keep my eye on the door, ready to speak up if i ever see the handle move. the responsibility goes both ways; but i don't think you can blame anyone in this case.
if it's a public bathroom or toilet, it's usually gender-coded, not much harm there as far as i'm concerned, so no big deal. if it's at home and we're family, no big deal, either. if it's the opposite sex that walked in on you and you felt embarrassed, then take it as a big learning experience to knock before entering ANY door, and to lock ANY door if you're about to do something private.

If someone's already in the bathroom, it's the responsibility of the person using the bathroom to lock the bathroom door once he or she enters it.

Not everything in life is someones fault and needs to be judged or blamed...
Yes the person walking in should have knocked and yes the person using the pisser should have locked the door.
Just say, "excuse me", and wait your turn and the person on the pisser can say, "I'll be right out!"
Don't sweat the little stuff.....

the stupid person who didn't lock the door. It's a bathroom and if that person has some brains within his/her head, he/she would've locked the door because someone might open that freakin' door in the first place!

i think the person going to the bathroom is at fault ... because they could have locked the door.. and other people would think if its unlocked.. that noone is in there

Are you all kidding? or did your parents not teach you to knock before entering a room? ALWAYS knock. I find it very rude for people just to burst into any room when the door is closed. period.
