The warden of the jail is authorized by the state or federal goverment for all aspects of inmate care. A doctor and/or nurse usually handle the medical care of inmates as agents of the prison/jail. In a temporary jail facility, there may not be a nurse or doctor employed by the facility. In these situations medical care is usually taken care of at a local hospital or other treatment facility. Inmates should promptly notify law enforcement personnel of any medications they require.
Note any medication given to inmates requries the permission of the prison doctor. Just because an inmate's personal doctor ordered it, doesn't mean the inmate will get it, especially if involves narcotics, stimulants, etc.

the inmate is responsible for the cost of their medications, the jailer/jail nurse is responsible for giving them to the inmate.

I would say whoever is responsible for passing meds everyday. I'm a nurse, and I know if I omit a med without calling the doctor first, it's a medication error & I have to write myself up.
