If you use the word "doctor" as a noun in the context of being a physician/healer, then the antonym is a person who is being healed such as "patient". Other books refer to the word "doctor" as an example of a phantom antonym, meaning one thinks there is an antonym to the word, but really there is none.
If used as a verb eg: to doctor people, then the antonym is to "hurt" or "damage" people .

depends on which meaning of doctor?

e.g. to doctor(change, which is verb) a report, then it will be undoctor I suppose.

I think verbs have antonyms, where as nouns usually don't
i.e. what the antonym of cat?

Alternate meaning of doctor is to change from original state or to tamper with, so the opposite that would be clean, distill, untouched or pure.

King Davin (above) was right. It is "patient." The doctor is the one who treats. The patient is the one who is treated. That is, the patient is just the opposite.

Injure or wound

If you use "doctor" as a verb--as in "She doctored my wounded foot"--the opposite would be "She wounded/injured my foot".

But I guess you could name someone known for harming rather than healing.
