The MAternal ancestry refers to your family or parents by MARRIAGE.
The PAternal ancestry refers to your family or parents by direct ancestry (birth.)
With the many (in the billions) of risks and "variances" of family lineages, the Donors have many considerations (as do you, I'm sure) to take before actual donation is made.
Yes, these conditions are quite highly researched and regulated (especially for situations that you are seeking to participate.)

Maternal refers to your mother's side of the family, and paternal refers to your father's side. Maternal grandparents etc. would be your mothers parents, and Paternal grand parents would be your father's.
