I assume you mean in Western Europe? There was no oral hygeine. Most towns had no dentist of any kind. Periodically a tooth drawer (to draw in the sense of, to pull) came through town. He had an assistant, a young boy, who banged on a drum. This was in part to "drum up" business and partly to drown out the screams of the patients as their aching tooth was pulled. In lieu of the tooth drawer, sometimes the local blacksmith performed this function since the tools were similar. Some cultures we think of as "primitive" took better care of their teeth than Europeans!
