First, they are stickers, and the design is not a star, it is a buckeye leaf. The Buckeye Tree is the state tree of Ohio.

Second, they are awarded one every time they have a good play. If the player has been struggling running a certain play in practice, but runs it successfully during a game, they are awarded a sticker. If they lay on a big hit, they get a sticker. If they get an interception, the probably get a few stickers, etc., etc.

The stars are miniature buckeye stickers, and are awarded to players for exceptional plays on the field.

They're not stars, they're buckeye clusters, and they're for making big plays such as sacks, TD catches and etc.
