you can call the hospital u were born at, the other if worse comes to worse, I have lost ID before and went and made an affidavid, my own version was that I wrote on a piece of paper Called a declaration, that I swore who I was where I was born my age my parents etc... and after writting down all the proof of who I was I then went to the clerk of the court, in the court house and asked for a stamp from the court on the document,

should also say that I ended the document (dated this day _____ the month of_____ the year of _______ in the city of ______ in the Province of ______ Canada,

if it does come to this I can help you along the road all the way to who to talk to.....

Call the hospital or township you were born in and go to the Vital Statistics Office they will have one. You need a photo ID and will have to pay for a new one. Good Luck!

You will need to contact the hospital you were born in. Hopefully they will have your records and be able to recover a new Birth Certificate for you. FYI it will be pricy to recover or renew it.

It isn't free, but since you evidently have a computer, go to your state's Dep't. of Vital Statistics, and follow the online instructions, and you can purchase one. I think they are 15 - 20 dollars for a certified copy.

You have to contact Vital Records in the county you were born. They will have you verify a series of questions, like your mothers maiden name, and such. For a fee they will send you a new copy that can be used for your license and such.
