Ba(OH)2 is a strong base which means that it dissociates 100%. Also you don't have a solubility problem for that concentration, so you have all of it dissociating according to the reaction

Ba(OH)2 -> Ba(+2) + 2OH(-)

from the stoichiometry it is obvious that the concentration of OH(-) is going to be double than that of Ba(OH)2, so the correct answer is d)6.4 x 10-03M
