The first symptom in most people is difficulty in voiding urine. There is a blood test called prostrate specific antigen or PSA that should be done on males over 40 also a rectal exam can feel a swollen prostrate. This should be a part of any complete exam by an MD or an APN. Late signs of prostate cancer can be it's spread to any other organ.

Well, sometimes it's asymptomatic, other times the presenting sypmtoms of the prostatic carcinoma are those of metastasis as this type of carcinoma tend to spread in different organs mostly Lung, so it might start with just some chest symptoms, like coughing of blood, difficulty in breathing....etc.
Rectal examination can detect whether there's a change in the prostate or not, it's done cilically without using any tools, just digital rectal examination(DRE).
If you suspect you have any signs of prostatic carcinoma, just don't waste your time and visit your doctor for further investigations, cuz it tends to spread and won't be managable if it spread.

You have a high level of Prostate-Specific Antigen (PSA) in your blood and the prostate gland itself is enlarged and, usually. harder than usual. See a urologist. (Your regular doctor can refer you to one -- this is a medical specialty). He can give you the blood test for PSA and palpate (the fancy term for "feel") the prostate itself. (It's between the bladder and the penis, but you have to know just exactly where.) The PSA is a lab test so it may take a week, but this is one nasty disease if it gets started -- it tends to metastasize, i.e., shed cancer cells which are carried by the blood stream to different parts of the body. It needs to be caught early, before it starts to do that.

A prostate operation will _not_ affect your sex life, at least after the operation soreness heals up. Some guys worry about that and let it go too long and wind up dead, which _does_ affect your sex life.
