It depends on the context. Some guys stare because they're afraid to approach a woman. Other guys stare because they are contemplating some type of rude behavior. Be careful.

Dont mistake Lust for Disgust. Guys stare at everything with a pulse. They might be wondering where you bought that shirt, or how quickly they can get you out of it. Most men want one thing only, and they stare out of curiousity or fantasy. If you see someone checking you out and you find them attractive, try talking to them, since most men do not approach women under such circumstances, and find out for yourself if they like you.

no, not always.
Guys stare at women all the time. They can't help it. It might mean you are pretty, you remind them of somebody, that you have a nice face or nice boobs. Could mean anything.
It does mean you are worth looking at though.
