We are not that close friends but I have known her for a long time. She had been trying to get pregnant for 10 years and now she's pregnant with twin boys. I kinda want to get her something that stands out a bit.

these gifts stood out when my wife gave birth to our daughter 5 mo ago:

stroller for twins
receiving blankets (they will use a lot)
diapers/diaper service
maid service (to clean the house)
personal chef (to make the food while on maternity leave)
dvd player plus one-year subscription to netflix

If you want it to stand out avoid the usual clothes and bottles. Trust me almost everyone will take clothes or something you can buy easily and doesn't take much thought. Clothes and stuff are very useful but typical. So go big get her some clothes, diapers, necessities, plus also something that will pop out is if you have pictures of her from her pregnancy arrange them into something nice and give it go her. That will show you put work into your present and its very meaningful and she could put it in their room. So give her some of the basic stuff and that, if you don't have pictures get her a double stroller. That will come in very handy for a while especially with twins.
