whats the process by which an Amendment to the Constitution is passed and ratified? where does the proposal begin and what stages does it need to pass in order to be adopted?

An amendment to the United States Constitution must be ratified by three-quarters of either the state legislatures, or of constitutional conventions specially elected in each of the states, before it can come into effect.

2/3 of each house of congress must pass it word for word. the president signs it. then it must be ratified by 3/4 of the states. each state that passes it in a referendum must be an "up or down" vote: 50% plus 1 (simple majority) It then becomes an ammendment. so far hundreds have been proposed, 27 have passed.
