No. Typically you just need 2 forms of id (usually a ss card and DL if you dont have those most banks will usually take a picture id and birth certificate or insurance card just another form of id to show them you are who you say you are) and enough money to open an account usually it is about 100$ the bank i work at now its 200 but all the others ive seen are 100 and if you are a minor you will have to have a parent sign on the account b/c you are a minor and cannot legally enter a contract..(so if you were to go into the neg. the bank could not go after you since you are a minor just your mom and dad b/c they said they would be responsible)
Edit added....
You will NOT get any interest or % of money on an account unless it is interest bearing account I.E. Money market accounts, Savings accounts etc not just a reg. free (or even reg) checking account.

no, but age limits are in place at most banking institutions which would require a co-signer who is an adult to be on the account at least until you are 18 in most states.
