I am applying to graduate school and I need three letters of reference from my undergrad professors. Do I send them a letter with all of my personal information (i.e.GPA, organizations that I am active with) or just let them write it however they choose, and request background info from me if needed?

If you are able, you should stop by and see them in person. Say "I'm applying to Super School for Grad Program and I'd like you to write a letter or rec for me. Is that something you could do?" Take a copy of your transcript or resume. They should be able to write a letter without any help, but it certainly doesn't hurt to offer.

If you can't see them in person, call them and ask.

If you can't get them on the phone, I think email would actually be best. That way they can just hit reply to get back to you. Much easier.

The site below is by far the BEST advice I've seen on asking for letters of recommendation.

Make sure to heed the advice about what to bring with you when asking for a letter. (As you anticipated, it includes a resume or list of personal information and activities, but also a bit more!)
