You'll need a passport, but if you only plan to visit for a few weeks you won't need a visa. You do need to already have a place to stay setup in advance so you can give the address when you go through customs. I have seen them refuse entry to people who haven't done this. You may also need to be able to prove you have enough money for your visit. And since you're just visiting, you should be prepared to show your return airplane ticket. Have Fun!

After January of 2007 if you are leaving the country and visiting outside of the U.S. you will need a current passport. If you are arriving via a cruise ship you have until January 2009.

No, but it depends on how long you want to stay there. You would need a visa if you will stay there for longer than 3 months, but I'm not entirely sure, they sometimes change the rules.

it is better to be safe than locked up by the police after a long flight and sent back!

if you're holding a US Passport, then you won't be needing a visa to enter UK
However, if you are just holding a green card and a non-US passport holder, then you may need a visa.
Visas depend on the passport held.
