I believe it means that when your drunk you tend to say exactly what's on your mind. Most of the time alcohol tends to curb your inhibitions and make you think you can say anything you want with little regard for the reaction of others. It is never that you didn't mean to say it, it is that you regret the way, tone, or perhaps honesty of the statement. Usually in the first stages of inebriation the truth comes out. If a person has a couple the truth comes out, more than that and there is usually no truth at all.

Alcohol tend to be truth serum for most people. When you drink you say things that you never would when you are sober. The things that have been weighing on your mind come out...sometimes it can hurt you in the long run because when some people get drunk they do not know when to shut up.

Since liquor loosens one's tongue, you can really understand a person when they are drunk, as his inhibitions are removed.
