The Hulk was one of Marvel's earliest classic "anti-heroes". He is not a hero as such, just a menace. They have him run into other bad guys and fight them so that he can be technically called a hero, but the Hulk doesn't have much choice about whether he changes. He has never been in control of it.
Secondly, about Hulk's shorts, there is something called the Comics Code, which was in its heyday when the Hulk was introduced. Nudity was prohibited. So they let his shorts expand as he did. Also, Marvel did a one-panel "What If the Hulk's shorts didn't expand", and it showed him blushing, standing in the middle of a pile of shredded clothing, covering himself up with his hands.
Also, come on-- after all, it's a comic book, not hentai...!!! Notice that while Hulk is muscular, there are also areas where he is NOT particularly well-endowed... such as in the social graces, intelligence and patience. (If YOU were that strong, but has a 'wee willie' that the ladies would sneer at, wouldn't YOU go on a destructive rampage...?)
Actually Mad magazine did a spoof on that. They showed Bruce Banner a woman (who was supposed to be his wife Betty, I guess) getting into bed. Then she slapped him, to make him angry. He began to change into the Hulk just as she turned out the light and in the morning she was shown with a big smile on her face. Those things are only played for laughs.
