Just go to your main game page and click on help in top right corner. Then click on what to do if a player is harrassing you and click again on abuse report form. This is what the form looks like:

Many websites have a report abuse button. Yahoo! Answers, for example, has a flag and text that says report abuse next to every question or answer.
