When a baby is born, and you pick the name, does it have to have the same last name as a parent, or can you just pick a random last name out of thin air like the first? Like the parents last name is Smith, can the babys last name be Jones? Just because?

Yes, but It is kind of confusing for the baby.
A lot of parents who don't want to give up their last name but they don't want their baby to have a heifine last name like "smith-jones" will combine the letters of their last names to make one. like "Jomith".

Yeah, it can be done. A baby can be given any name, and when they are older they can also change their name to whatever they want.
I would perswonally want to share the same last name with my children. I would find it a bit if strange if someone chose to give their newborn a completely different last name, but no one can stop them.

You know I never thought of it. I know someone with the last name Belch and the father took on the mother's name when they wed, and I have to say it was quite decent of him so now his kids arn't named Whatever Belch. I would want my kids to have my last name, but then again my last name isn't Belch.

You can name your child any name you want including the lastname (Sir name)...
A child is not required to carry either parents lastname(Sir name)...
Traditionally the child carrys the father's lastname(Sir name) even when the parents are unmarried... And his name is listed on the birth certificate...
The child traditionally carrys the mother's lastname(Sir name) when a father is not listed...
