Picking a costume for a young one is a hard thing to do. Take her to a costume shop. Guide her and help her pick something out. Here are some suggestion:
-Angel
-Animal
-Fairy
-Witch
-Nurse
-Model
-Her favorite television character (ie. Barney, Dora the Explorer, etc.)
Search some costume stores online and see some choices from there.

the disney store has a bunch of cute costumes. various princess costumes, the little mermaid, belle, princess jasmine, cinderella, mulan, etc. she can pick anyone she wants and they're all very cute and appropriate for church. trust me i went to a catholic school so i know a little bit about dressing appropriate for halloween dress up day. good luck!
