It depends on the government bureaucracy in that third world country. However, it is safe to assume that it will be tough -- especially if you are a foreigner trying to establish an orphanage. There will be suspicions on your intentions for one (e.g. if you are planning to sell babies).

Make inquiries with the Department of Social Services or whatever the counterpart agency in a third world country of your choice. Or check with missionaries and religious organizations that operate orphanages in third world countries for information

It is not really difficult to set it up as long as you can get the government to issue the permit and you have allot of financial aid to back you up.
It is a huge expence to run an orphanage anywhere but especially in a third world country. Just the cost of medical bills alone would be very high not to mention food, clothing, beds, bedding, hydro, water, sanitation, laundry,and any regulation fees you need to pay, along with security people and other staff.
This would be something you would ahve to really look into.
