I want to use it to get time off from my boss. Plus I told my girlfriend my back is out so she will shovel the driveway this winter. A note would help to get these nags off me.

You're making your girlfriend do work based on a lie? Some relationship you have. I hope there's nothing out there to help you with that. If you need a coupls days off that badly, just use up your sickdays. If you don't want to, then live with it.

if you get caught you will be going to prison for a while it is a felony, forgery of dr script is serious than add forging a fake signature. you are looking at a few years. do not try it.
