Most definately the hook through the cheek thing...closely followed by Preston crapping in the farting helmet and Steve-O throwing up all over himself!!

Most definately, the one where he didn't know that his beard was made out of pubic hair (not to mention the crabs in it). And then it showed him taking the pubes out of his mouth and laughing about it! YUCK!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! I consider myself to have a pretty good stomach, but that made me want to throw the hell up! EW!
The beer-bong up the *** was a close second, though.
