Generally, when an organization or individual, such as a school district or church decides that a book is bad and they choose to not allow the book in their library or its use as reading material, it is considered banned.
An example would be certian perocial schools do not allow the "Harry Potter" books to be read for book reports. Those books would be "banned" from their reading lists.

Banning an item is censoring it from the public view. In the belief that it will cause great harm, either physically, mentally or other to a person or persons.
Banning comes from groups of people, religious, political, or educational. They believe that they are sheltering people from what they believe is wrong.
Personally I believe that if you are a mature person you have your own mind to make up in whether to watch, read, or listen to something. As a mother, I have that right for the sake of my children.
Be careful of what is going on around you. There could be more pitfalls than you know.

A banned book would be such books that certain groups/schools/etc. would not like their readers to read. For example some books that are graphic (sex, foul language) wouldn't be appropriate for school aged children, therefore they are banned from their library.
