It depends completely on the discipline and on the particular exam. In advanced mathematics class, it would probably represent the highest grade in the class. In sociology, it may well be one of the lower grades.
In most finance classes that I have taught, it would represent a low to mid B (3.0)

The following is a general percentage/letter grade scale for classes taken at U.S. colleges:
100 – 90% = A
89 – 80% = B
79 – 70% = C
69 – 60% = D
59 – 50% = E
49 – 0% = F
A = 4.0
A- = 3.7
B+ = 3.3
B = 3.0
B- = 2.7
C+ = 2.3
C = 2.0
C- = 1.7
D+ = 1.3
D = 1.0
E or F = 0
