It depends on the area. You can call the facility and ask them. They may have you contact the doctor. What they usually need to give you a quote is: 1. the CPT code of the procedure (every surgery has a number assigned to it called a CPT code) 2. the length of time the doctor has requested (this is usually a "set" time for and D&C. They should be able to give you a quote with this information. The anesthesia charge will be separate from the operating room charge. You can get this from the anesthesia office.

The costs vay by parts of the country, so any answer anyone gives you as to exact price may be high. I live in Alaska. Medical costs are higher here, so even if I knew the exact cost, it would not help you.

I presume you mean a D&C (dilation and curettage). Actually, this is usually not performed in an OR, so that reduces the cost somewhat. Light general anesthesia may or may not be given.
This can be performed at a clinic, it can also be a form of abortion procedure. Depending on the requirements, my guess would be anywhere from $500 to $1500 for a simple D&C.
It could be double that depending on the institution and whether they use a separate fee for an anesthesiologist.
