1. Get your book completely written. Do not query a novel before you have finished it.
2. Go back and reread the manuscript and revise.
3. Consult Writers Market to find out what publishers are looking for. Pay attention to the length they want and how they want it.
4. Look at your manuscript again. Prepare it according to the publisher's guidelines. INCLUDE A COVER LETTER.
5. Submit your manuscript.
6. Start your next book. It'll be a long wait before you hear back on the first one.

Check this website. This guy, Woody Young, has quite a number of books on the process. Two of them give listings and references of publishers, manuscript screening services, and most importantly, the introductory book takes you through the process.
