'Referral' means 'how the patient got to see the cousellor' (or whatever professional they're seeing).

'Referral procedure' means a method of deciding whether the patient actually needs counselling (or etc.), whether they need to see a different sort of professional, or whether they need some professional help at all.

The idea of having a procedure is to get people the right help when they need it, instead of just trying things out randomly. You will have your own ideas about the important issues that arise from any of these possibilities.

A patient finding a counsellor, medical specialist, herbalist, hypnotist, physiotherapist, psychiatrist or etc, for themself is called 'self-referral'.
