to stay upon entry. Is there some underground means to extend the stay legally?

I'd like to hear any and all experiences from those who have spent some time in Bali.

It's called the Holiday Villas, you need an agent to provide you with the necessary "Invitation Letter " to be included with other documents required when applying for the visa.
Otherwise a lot of these foreigners have local friends that pretend to "hire" them so that they can have the work visas/KITAS.

Some of them travel to other places using Bali as a place to go back to, while on trips they have their visa renewed, otherwise they get visas for investing, working or an invitation from a Vacation villa.
Underground usually means illegal so I wouldn't go there, if you do want to extend your stay in Bali I suggest you look for serious answers at an Indonesian consulate.
