do colleges really care is someone is a national merit semifinalist/finalist/winner? how much of a difference does it make?

YES being a semifinalist or finalist matters for admissions. You will find yourself heavily recruited; finalists are generally in the top 1% of students and one of the ways in which the quality of schools is rated is by the quality of students enrolling. Thus the number of NM Finalists entering the school is an indicator of the school's attractiveness and the high level of selectivity or competitiveness it takes to enter.

I was a National Merit Commended and several schools offered scholarships in large part because of that ... my grades were'nt all that terrific (not bad just not outstanding). But my test scores (including the NMSQT were high)and that did the trick...

unless it is a really small private college, they really don't give. seriously, it means nothing.
