When an offensive player crosses the line of scrimmage before the ball is snapped. When a defensive player does it, it's called "offside."

A false start penalty is called when an offensive lineman moves after setting in his stance. The result is a five yard penalty against the offense. It's important to note that the move doesn't have to be significant, often a flinch by one of the lineman is all it takes to draw a penalty. The purpose of the penalty is to prevent the offensive line from trying to trick the defense into coming across the line of scrimmage before the ball is snapped, by flinching or moving out of their stance. This is why defensive players that jump offsides usually point fingers at one of the lineman, to show the refs which guy they think flinched.

A significantly less frequent occurrence - A quarterback can be called for a false start penalty, even though they are allowed to move and gesture. The false start penalty is called when the quarterback stands behind center and does a move that simulates snapping the ball, without calling for the ball to be snapped. The QB can try to draw the other team offsides with his voice only, but when the QB uses his voice and overt actions that look like he is snapping the ball, the refs will throw the yellow flag.

Remember:
False start = Offense
Off sides = Defense
Play is dead before the ball is snapped = false start
Refs will let the play continue = offisdes

The way to tell whether the flag that has been thrown is due to an offisdes or a false start is to look to see whether the refs let the play continue. If they blow the whistles and stop the play immediately = false start. If they let the play go till the end = offsides.

A false start is when you have to take a dump, you think you have to fart, but in reality, some poop falls into your underwear.. So it was a "false start"
