deviation is soft word than problem. You can easily communicate others that you deviated from the rules, regulation or policies of the company but if you direct use the work "problem", people can anger and it is not a good way to communicate. You can understand it like that you are going to communicate "Bad News Message" and it should be in a more professtional manners.

Because people don't like the word "problem"... it suggest something harder than a "deviation." It's just a play on words.
