how to install low voltage transformer and chock?

and chock? can't help you there, but the transformer has two screws marked 110v. take an extension cord, cut to size, attache the wires to the screws. The two screws on the bottom will have app.16 volts, attache on wire from the doorbell(?) to one of the screws. Attache one of the wires from the chime(?) to the other screw, fasten the other wire from the doorbell button to the wire from the chime
