Either there is no game in it, or it isn't reading the data. blow any dust away, make sure the disc is clean, turn the unit off to make it reset, and it should be ok.

Sounds like it may be a hardware failure. Have you tried completely powering off the system (even disconnecting the power) and re-powering it back up (without a disc inside) to see if it still occurs?

it might not be plugged in the TV
