Gently, softly, and with great care. You don't say whether or not this is something that could hurt them. If it is, be ready for the blow back. Take it calmly and keep your head.
Don't write it. Notes do not convey tone of voice or intent.

Telling someone you love something that you know will hurt them is hard..there's just no easy way to do. Simply sit them down, look them in the eye and get it all out into the open.
