I think that the order form would be a "type" of record in a database.
Maybe it's called a "Class" and the records in a database are differentiated by "type" or "Class".

A single order form could be thought of as one part of a customer's file. Thus, if you are putting the order information into a computer, the order form would become a single record relating to that customer.
