My son is now just shy of 4 months so I know what you mean about being at a loss. When you look at the registeries, it's rather confusing and the stores would have you believe you need everything on the face of the planet! Go for the basics first. You can always add on later:
Clothes:
Sleep'N'Plays are awesome. They're a lightweight t-shirt material with legs and long sleeves.
Onesies - Long and short sleeve.
Heavier sleepers or sleep sacks if you're in a cold climate.
Socks (forget the booties, they don't stay on very well).
Stocking caps (if you plan on taking your baby out on a regular basis).
Bedding: Decide where you're going to put your baby for the first month or two. We used a "Play'N'Pack" with the bassinet feature for the first three months (until he started sleeping through the night) so we could have him in the room with us. If you're going to put your baby in his own room, then you'll want to decide on your crib. Regardless of what you decide to go with, you'll want:
Waterproof matress pad
Sheets (at least three to start with)
Blankets (Frankly, I got a ton of recieving blankets which I've hardly used other than to put over his rocker and swing seats).
Baby monitor if you're putting him/her in their own room.
Bath:
Invest in a baby bath. It will save you a lot of headache and backache!
Baby hair and body wash.
Hoodie towels.
Wash clothes.
Misc.:
Diapers (newborn and size 1)
Wipes
Diaper rash ointment
Baby safety kit (hair, nails, thermometer, etc.)
Pacifiers
Infant carrier
PATIENCE!

1 grooming kit (brush and nail clippers)
1 pack cloth diapers (use them for burp clothes or if you baby gets a bad rash)

2 dreft laundry detergent (very inportant for their sensitive skin)
2 shout pen to remove spit up stains (one for home, one for diaper bag)
