Seems an odd question -- what was I wearing? - but here goes. I've had them and I've given them. You wear the clothing you come in with, you expose the buttocks and are asked to stand at the end of the exam table, place your hands on the table, lean over and place one foot up on the step. This helps to relax the gluteal muscle making needle insertion easier and less uncomfortable. The pain of the injection itself varies with the particular skill of the one giving the injection but it's the medication being injected that is far more likely to hurt. It's no big deal.

Wear something easy to pull down and up. Elastic wastebands are great but not necessary. Bathe well before you arrive. I have given and received many shots in the buttocks. When they are given properly and quickly they are almost universally painless. When there is pain, it is usually because the medication is an irritant or it is being injected too quickly.
You'll be fine. Make'em give you a sucker and a sticker.

I give myself shots in the butt. I have to take an injection every two weeks. I usually give my injection after I come out of the shower. It isn't all that unpleasant. It pinches a little, but not anything to yell about. You can wear normal clothing. You'll only need to expose the upper outer side of either cheek. I can say it isn't as bad as it sounds.

I got a penicilin shot in my buttocks when I first came into the army in 2000. It hurt really bad and was a hard lump, the size of a golf ball and couldnt sit or sleep well for days.

the shot felt better than all the vomitting i was doing all night and 1/2 the next day. the medicine made my gut and stomach come back to normal position. well I had to pull my pants down so the med staff can get to the target.

It hurts the day after and not the day it happens. Hurts like hell the next two maybe 3 days
