In elementry school they hung sheets in a sort of maze with people chacing after you dressed up as Freddy Kruger and such. (I had never been scared of him untill then). You could do something like that , and have paper meche slabs of meat hanging from the ceiling.
I think the music should be scarry too. Don't use the typical Halloween tapes/C.D.s Try something like the beat from, Friday the thirteenth or Halloween, maybe even that creepy sound from the beginning of Jurassic Park.
People dressed up as zombies and what not aren't really scarry unless you have the money to spend on better make up.
Puppets are also kind of creepy.
Don't forget a fog machine or dry ice.

Well, depending on the age group you are referring to, you will ahve different things. So carefully consider your "audience" carefully. A man with a fake axe jumping out at a group of preschoolers would be a very bad thing.

Some of the best ways, and perhaps simplest is to use sound effects to their greatest advantage, not to mention fairly inexpensive to create. The right mix of sound and shadows can invoke that feeling of anticipation get the andrenaline running. It may be quite interesting to have a few rooms that build up to the real scare by using this type of ambiance. Stuff like scraping sounds, creaks, heavy breathing, etc.

Why not have different rooms patterned after differnt scary movies like Chain Saw Mass, Chunky, the Grudge, etc. Then they have something they relate to.

in one room, have a skeleton in a coffin, then when someone steps on a device, it pops up squirting fake blood on them:-)
