Many guides say that measurement should be done with your bra on, but if you are currently wearing a bra that is too small then a measurement taken with it on will be inaccurate, since your breasts will be constricted. If your breasts are not too large then measurement is best undertaken bralass. If they require some support then choose the bra with the least restrictive cups.
Using a soft tape-measure1, firstly measure around your chest under your bust, at the level where the lowest part of your bra should sit. Try not to let the tape slip down at the back, it needs to be level all the way along. Take this measurement (in inches): if it is even add four, if it is odd add five. The resulting number is your band size.
Next, measure around the fullest part of your breasts, don't squish them unless you want your bra to squish them too2. This measurement in inches is your bust size.
To obtain your cup size, subtract your band size from your bust size. If the two are the same, you are an A cup. If your bust size is 1 inch larger than your band size, you are a B cup, 2 inches larger and you are a C cup. This continues through the alphabet, though not in a strictly linear fashion, due to the presence of cup sizes such as DD.
Bras are bought by band size and cup size, for example 34B, 36C. Easy!

The first measurement comes from just under the breast all the way around. That's the number measurement 32,34,36,etc. To get the cup size you measure through the center of your breast all the way around. For each inch above the original number you add a letter. So if your first measurement is 34 and you get 36 for the second measurement you would be a 34-B since your boobs stick out two inches from your body. That's how you find your bra size.

if this measurement is an odd number +5 inches
if it is an even number +4inches
2,CUP SIZE measure over fullest part of bust making tape is level
if the measure from 2 is the same as the created measurement of 1 then u buy an ,A .cup with the measurement from no .1.
if the measurement2 is 1inch or more than back size but a .B. cup
if the measurement is .2inch.then a .B.cup etc etc

I say, try it on.. If your breasts are bulging out too much then go up a cup but if they fill the cup and just stick up a little then perfect. Now as far as around, whatever feels best to you is the best bet. If your heavy chested you may want the strap to be larger. If smaller chested the you can get away with wearing the smaller straps...... I have been wearing bras for years and never has a mishap yet!

The common method described by some of the answers above doesn't necessarily work for everyone (the fit depends on your body shape, not just size). Also, different manufacturers have different sizing systems. Your size in one brand might be too small or too big in another. You really need to try them on.

first mesure below the bust add 5 inches if odd and 4 inches if even
then mesure around the bust subtract that number from the other number if they differ by:
0-AA
1-A
2-B
3-C
4-D
for example:
Jens first mesurment was 30+4=34 34
Jen next mesure ment was 32 -32
-----
2
so Jens bra size is 34B

if your really worried about your size then get measured, I suggest marks and spencers but if you are a big cup size then you need to get measured in what ever shop you are going to buy from, because they do vary slightly. I tend to buy from figleaves.com who also have a page telling you how to measure yourself.

I would just go to Victoria's Secret and have a salesperson measure you...they are trained to do so and know exactly what their doing!!!
