I would think it would be quite awkward for an American doctor, born and raised in America to speak with an English accent. Don't you?

i don't understand why human beings replace their accents. it is a similar element while an American places on an english accessory. The accessory works for the character yet then in different issues the actor in simple terms sounds extraordinary. i'm getting used to listening to one element and while they modify it, i'm getting thrown.

Because the writers, producers, and directors of the show wanted the part to be played by an American, but they loved Hugh Laurie when they saw him. It is explained in the bonus features of the season one DVD set(which i have and have watched many times).

It's probably because there is nothing special and surprising about a snooty brit (if anything, that character is more of a norm associated with the British). Seems an American character would be more interesting.
