Not sure where you are, but check out http://www.bravissimo.com from the UK. I have a friend with G cup breasts and she swears by this company.

Lane Bryant. I have really large breasts too, but am smaller around, so it's REALLY hard for me to find any clothes or bras that fit. I'm 32DDD (some brands I'm an F! What the hell?). Anyway... I did find some button-up shirts that have this sort of scrunchy elastic part to it... I think I got them at Le Chateau, but that was years ago and I don't know if they still sell those. They were great though because the bunching was a part of the pattern and it looked really good on big-breasted women. There are some corset-type clasp up shirts that I got at H&M, but again, it's a different season now, so I'm not sure if they are around anymore.
Anyway, try Lane Bryant, they have clothes for larger-breasted women as well as larger women.

My breast are a C cup so I can't really help you with where to go for shirts. With button down shirts I have that problem of "gaping" too. What I do is wear a tank top under it and then only button the bottom 3 buttons. It's a little tip that hopefully might help you.

I have the same problem, I am 38DD and I have to buy shirts that are big because of the buttons gapping and when I do buy big shirts they make me look really fat. I have been buying clothes from Cato's for awhile now and they have cute clothes. Check them out. (And not too expensive)

I understand, I am a 36DDD, have you tried stores like lane bryant, their clothes are very nice, or maybe the larger department stores might have botton down blouses that will fit better, I have the same problem, I usually stick with tee-shirts and sweaters and sweatshirts.

Well I dont know where would you find such clothes, but i can certainly tell you what to wear if your breasts are heavy. Go in for tops with vertical stripes and dark striations. Preferably wear tank tops with lose pleating around your breasts. And chose a Minimizer bra with firm back band and full cusps.

If your insurance pays for a breast reduction operation you have you problem solved and you probably will look better anyway, and the money you have spend in the operation you will save it on clothing and really elegant posture. Thanks

PS: also YOUR BACK WILL BE GRATEFULLY TO YOU AND WILL BE STRAITEN OUT WITHOUT THE PAIN.
