No, the mother must have the approval of the father unless he has commited a felony or disowned the child.

If you used the father's last name as the babies on the birth certificate, then no. You have to get a Court Order allowing the name change. Call a family court lawyer and they can give you the specifics for your state.

Not if he is on the Birth Certificate! The court can change it without his consent, but there better be a good reason! Usually names are changed when deadbeat dad takes off, his rights are terminated and a new birth certificate is issued! Or he is convicted of some heinous crime!
