I can assume from the question that a 'formal carer' has a set routine.
8am wake the person and get them dressed and cook them some breakfast.

An 'informal carer' visits the person at 8am with no agenda does whatever the person requires at the given time, whether it be shopping, cleaning, etc.

As it Says informal is not the run of the mill career a formal career is a paid career following strict guide lines by employer so a formal career is employed a informal career is some doing it free as said by goodbhoy Dave

I think the main difference will be if they get paid for it or if its voluntary
