You say Hi! How are you? because the exclamation mark (like the question mark) is a full stop with a squigle above it. just like colons too. ( : ) but not these ( ; ) they have a lower case letter becasue there is a comma at the bottom.

I think the second is correct. I would always put a capital after exclammation or question marks.

The last one I think! The quality of spelling, punctuation and grammar these days has so deteriorated that no one really cares where the exclamat!on mark is put.
