The most common instance of breaking glass considered to be an ill omen was breaking a mirror. There are two theories as to why it is bad luck to break them - one was because the gods were considered to contact you from the world within the mirror and the other was that the mirror contained your soul.
It is also considered bad luck to break a glass bottle - these were expensive items in the past.
Finally, it is considered bad luck to break a glass when proposing a toast - obviously because this was a bad omen to those who you were toasting.

Breaking glass is only an ill omen for some cultures.
In Germany it is tradition to smash plates, glasses, and porcelain at wedding to bring the couple good luck. At my Cousin's wedding a toilet was smashed among dozens of other glasses, plates, and such. They are still together and happy.
BTW: The couple is expected to clean the mess as well.

It isn't considered to be a bad omen unless you give way to that kind of thinking and allow yourself to believe in certain superstitions.
