Look, you had enough courage to ask the man for a job in the first place. You should be man enough to tell him what's happening and resign. If he gets mad, so what? You gotta take care of your family.
You can always get another job, but you cannot get another family.

Face-to-face is always better, but I'd also follow-up with a written resignation. Responsible thing to do -- you may need to apply again if you return to the country or at least get a reference.

Tell your boss the situation first. With "family emergencies" they usually try to be understanding.

Explain your situation to your boss. He/she may be flexible with you (assuming you really don't want to quit). Even if you do, it would be unfair to your boss and other co-workers to not provide advanced warning, whenever possible.

This doesn't seem like a situation in which you would need to burn bridges and, who knows, you may need this person some day for a reference or re-hire.
