It matters if a payment is going to be paid to informal carers, such as a cash benefit from the taxpayer..... or a tax exemption, which is also 'really' a payment.

It is difficult because there is no clear & logical place to draw the line. If I call in on my grandmother on my way home from work every evening, am I her informal carer? If I give her her pills so as to be sure she remembers them? If I phone her twice a day? If I spend an hour with her every day? Two hours? Six hours?
