From what I remember, a disturbed man killed his wife and son through a fit of rage, which unleashed an evil spirit that killed him and anyone who entered that house.
Gives me the chills just thinking about the movie. The original Japanese movie "Ju-on" which "The Grudge" was based on is a lot scarier, but just as confusing. :-) You should watch it!

Its not that confusing if you just pay attention. The japanese girl liked her professor (Bill Pullman). She sent him love letters and kept a journal about him. Her husband found the journal and went crazy, killing her and then her son. In Japan, when a child is killed and a cat is killed at the same time, the child will take on catlike qualities. That's why he meows. When the husband killed the wife, her neck was messed up so she makes a gurgle sound. The american family that moves in is hurt because they just got in the wife-ghosts way. They didn't necessarily do anything wrong. This movie is a remake of Ju-On.
